About the Practice

Cobblestone Health Care - originally known as Farmington Family Practice - was established in August of 1989 by Laurel Dallmeyer, MD. Dr. Dallmeyer wanted to establish a small practice with a high standard of care and a very friendly, personal environment. Since its inception, the practice has an excellent reputation, and attracts as many patients as desirable primarily through word of mouth. Over 10,000 patients make Cobblestone Health their medical home.

The current staff of physicians and nurse-practitioners includes Dr. Dallmeyer, Dr Shereen Gamaluddin, and FNP Nancy Allison. Nurses, receptionists, a billing specialist, and an office manager provide excellent support services for the providers and the patients. Most staff members have been with the practice for over 5 years, and know most of the “family” of patients by name.

We provide many services in addition to primary care visits, so no matter your health care need, give us a call. We may be able to save you an emergency room or specialist visit!

We perform routine adult, gynecologic, infant and child care, repair of lacerations, treatment of many injuries, removal of moles and warts, evaluation of elderly for services, camp and school physicals, allergy shots and other procedures.

We are also able to see patients in the evening through Thursday, and on Saturday mornings.

Dr. Dallmeyer and her staff have provided over 150,000 visits to the people of Ontario County in the past 21 years, and continue to deliver the same comprehensive care for people from the youngest infant to the oldest grandmother. Our new location in the former Heritage Home for Adults is welcoming and convenient. Completely renovated, the waiting room has 2 original fireplaces and period light fixtures. A model H-O train, the joy of Dr Dallmeyer’s grandfather, makes a circuit of the waiting room ceiling, to the delight of children of all ages.
